The Process of Enrollment

Do We Need a Referral?

For participants using programs like Medicaid, a referral is needed from their case manager before we can begin the enrollment process.

If you won't be using Medicaid, we can skip to the next step.

Physicians Orders Needed

Once the care team has decided to move forward with enrollment, the potential participants Primary Care Physician will be sent orders to sign. Once those orders have been signed and returned to us, we can start attendance!

Initial Assessment

You will make an appointment with our Social Worker for an initial assessment. At that appointment, we will ask medical and social history questions and have you sign paperwork. The potential participant and a loved one or legal guardian should attend this appointment.

Arrange a Weekly Schedule

Circles of Caring will be in touch with you once Physicians Orders are received to establish a weekly schedule, duration, transportion options and when the first day of attendance would take place.

Care Team Meeting

Once the initial assessment is completed, they will bring that information to the care team at Circles of Caring (RN's and Executive Director). It will be decided there if the potential participant is appropriate for admission.

Begin Attendance

Everything is now in order and we're ready to go!
